A buttonhole includes a single flower as the central element. Sometimes, there can be a bit of foliage to complement it, but still, the main accent is this single rose or carnation or any other flower.
A boutonniere always consists of a cluster of flowers or other elements. It looks like a tiny bouquet of its own. A can be a bunch of florals, fruits, herbs, etc. But it’s always an arrangement of items, not a single item.
